Licensed Practical Nurse (Pain Clinic)

The Pain Management LPN collaborates closely with the pain management provider and team, including but not limited to, the Pain Management Provider and RN. The SCC LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The LPN demonstrates active collaboration and communication with the Core Team (Patient, Specialty Care Provider, RN Care Coordinator and the Clerical Associate) and the Expanded Team (family/caregiver, internal and community based services involved in providing care to the patient). The LPN must have the ability to react to situations in accordance with established policies and procedures, and ability to appropriately use this knowledge to work effectively with a wide range of patients. Additional functions the SCC LPN will be proficient/independent in include:
Utilizing all available tools, such as care management tool, to enable effective and efficient identification, intervention, and follow-up of individual patients and cohorts. Ensuring appropriate evaluation and access is provided to patients assigned to specialty clinics as assigned. Functioning at the full extent of the team member's relevant clinical privileges, credentials, scopes of practice, elements of practice, certification, functional statement, position description, or other VHA or local facility approved documentation of competency. Participating in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients. Technically proficient in initiating, performing and completing care of the veteran population Recognizes and reports unsafe equipment, conditions, and practice. Actively seeks out educational opportunities to enhance nursing knowledge and skills, and shares with other staff to improve and advance nursing practice. Perform specialty care operations management processes, as appropriate independently with little direct supervision (including the ability to organize work, follow instructions, write and type legibly and accurately). Perform communications and facilitate safe transitions of patients between the SCC site of care and other health care settings, using informal and formal communication methods, as appropriate. Providing health education and health coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and personal support persons, commensurate with the documented expertise or professional training of the SCC member. Engaging patients in using health care, encouraging patients to engage personal support persons, receiving input from patients and personal support persons regarding VA care. Using formal and informal communications that are respectful, effective, timely, and bidirectional with all team members (including the patient and personal support persons) to convey significant, clinically relevant information for the care of the patient. Collaborating with informatics technology staff to develop and implement systematized, electronically supported, standardized, tools to support SCC care delivery processes (e.g., pre-visit reminder calls, post-hospitalization follow-up calls, recall scheduling procedures, new patient orientation). Serves as a preceptor in orienting, educating, and training less experienced LPNs and NAs/HTs related to support duties for more complex, specialized tests/procedures. Documents in a timely, accurate, and concise manner. Performs daily duties and quality controls including but not limited to:
Gather information from non-scheduled patients such as a chief complaint and history of symptoms to document and communicate information to the Registered Nurse. Check patients in for appointments and gather vital signs. Orders labs and diagnostic tests appropriate for the LPN scope of practice following policy/SOP. Set up for major and minor office procedures/assist and chaperone Daily inventory for specific items on hand Maintaining contact with needed departments for supplies (Biomed, decon, processing, SPS, pharmacy, etc.). Track the progress and flow of equipment going in and out of the clinic Ensure equipment is in working order, f/u for timely repair/replacement if needed. Assists RN in consult follow up and scheduling patients. Monitors surgical patients ensuring appropriate scheduling for follow-up. Specialized
Experience:
At least one year of LPN experience. Interest, flexibility and experience in all types of practical nursing. Good interpersonal relationship skills and ability to work with multidisciplinary health care team. Strong computer skills. Graduation from an approved school and one year of experience that involved nursing care work in a hospital outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures. OR (c) Education:
Graduation from an approved school of at least 24 months duration. GS-05:
Completion of at least one year of qualifying experience at the GS-4 level or equivalent and fully meeting all performance requirements for the GS-4 LPN/LVN. 1. Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes. 2. Demonstrated ability to serve as a responsible member of the nursing team and interact in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice. 3. Knowledge and skill to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to RN or MD/DO. 4. Knowledge and ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO; and assist in resuscitation procedures in cardiac and/or pulmonary arrest. 5. Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action. GS-06:
Completion of at least one year of qualifying experience at the GS-5 level or equivalent and fully meeting all performance requirements for the GS-5 LPN/LVN 1. Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. 2. Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for appropriateness and conformity with established policies/procedures. 3. Ability to observe, identify and respond to the patient's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors. 4. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO. 5. Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed. 6. Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff. 7. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
